Editor's Note

This week's report covers 46 establishments. Four were closed or ordered to close for re-inspection, three of them tied to cockroach activity. I think Route 66 diners saga is finally over but again training issues and dirty sinks got them after reopening. Royal Hyderabad is frequently mentioned in these reports and think they almost always have follow up re-inspections.

Closures & Major Violations

These establishments were closed by inspectors or received violations serious enough to pose an immediate public health risk. Violations in this category include evidence of pests, improper food temperature control, contaminated food-contact surfaces, or conditions that create imminent danger to public health. A follow-up inspection is required before any closed establishment can reopen, and some may face additional scrutiny or civil penalties.

A1 Wings and Seafood, 6904 Menaul Blvd NE, Suite C
Inspected July 20. Sanitizer buckets tested at zero parts per million on the main line and above 200 in back prep. Cold holding failed across the board: raw meat at 52 degrees, cut lettuce at 55, sliced sausage at 52. The July 20 inspection was Unsatisfactory with re-inspection required. A follow-up on July 21 confirmed the cold holding unit could hold at 41 degrees, and the facility was returned to Approved.

Curry Leaf, 6910 Montgomery Blvd NE, C
Inspected July 20 after a substantiated complaint. Substantial cockroaches were found dead under the dish machine and in the dining room, plus rodent droppings above chemicals on shelving. Dish machine sanitizer read 10 parts per million; the only kitchen handwashing sink had its drain taped over and no cold water. The facility was closed. A July 22 re-inspection cleared enough violations to reopen under an Unsatisfactory grade, with one prep table still unable to hold cold.

EAT MY THAI CUISINE, 5303 Gibson Blvd SE
Inspected July 20. Cockroach activity was found throughout the kitchen, including the wok stations, dry storage, prep tables, and boiler room. Date marking and cleaning problems from two prior inspections remained. The facility stayed closed and must complete a compliance plan and staff food safety course before re-inspection.

Golden Chopsticks, 12251 Academy Rd NE, 104
Inspected July 21. Raw chicken was stored directly above cooked chicken, and raw meat sat on top of lo mein noodles in the walk-in. Raw chicken boxes were being reused to hold cooked chicken. The facility was closed for re-inspection. Corrections on July 23 returned it to Approved.

Limra DBA Royal Hyderabad, 5760 McMahon Blvd NW, Ste B-E
Inspected July 24 after a substantiated complaint. A pan of cooked chicken sat under water dripping from a ceiling vent in the walk-in freezer. Cold holding failed on tofu, sauces, and raw chicken left out at 53 degrees; the freezer itself read 60. Ceiling tiles were missing and last recorded temperatures were from June 19. The facility was closed for re-inspection.

Long Cao LLC DBA Dragon Sushi, 6904 Menaul Blvd NE, Ste D
Inspected July 20 and closed for a severe cockroach infestation. The facility was directed to hire a licensed pest control applicator. A July 23 re-inspection found three live roaches and reopened it under an Unsatisfactory grade.

Conditional Approvals

These establishments remain open but were given conditional approval following their inspection. They were found to have violations that, while not immediately dangerous, require prompt correction. Each must address the identified issues within a defined window set by inspectors. Failure to correct the violations within that timeframe triggers a mandatory re-inspection and may result in conditional status being revoked or the establishment being closed.

66 Diner, 1405 Central Ave NE
Inspected July 22. Caked debris and black matter coated the ice machine shield, employees were not washing hands between tasks, and walls throughout the kitchen had holes and cracks. Corrections were verified July 24 and the grade was raised to Approved.

D's BBQ LLC, 1200 Wyoming Blvd NE
Inspected July 23. No certified food protection manager, no hand drying at either sink, and the permit had expired. The facility must pay for and post a valid permit.

NA-TEAVO, 10000 Coors Blvd NW
Inspected July 21. Sanitizer tested at 200 parts per million, neither handwashing sink had hand drying, and the service area hand sink had no hot water. The permit expired November 30, 2025 and must be paid.

Little Butterflies Learning Center, 11216 Phoenix Ave NE
Inspected July 24. Tomato sauce and turkey were held past the seven-day limit and discarded. No certified food protection manager on staff. The grade was raised to Approved the same day.

Sticky Rice, 7600 Jefferson St NE, 5
Inspected July 21. Raw chicken was thawing in standing water with no running water. The employee turned on cold running water during the inspection and the grade was raised to Approved.

SureStay Hotel, 1635 Candelaria NE
Inspected July 20. Chopped green chile had no date marking, and a hand sink was blocked by a trash can. Corrections were verified July 22 and the grade was raised to Approved.

Sushi and Sake LLC, 4214 Central Ave SE
Inspected July 24. Dish machine sanitizer read 25 parts per million, and peelers, strainers, and pitchers were stored soiled on clean racks. No temperature logs were available.

SW Convenience Stores LLC, DK #707, 10324 Menaul Blvd NE
Inspected July 21. Sanitizer read zero parts per million, milk was on the display cooler dated as far back as May 30, and retail stock was not rotated first in first out. The sanitizer violation was corrected on site and the grade was raised to Approved.
