Closures & Major Violations
These establishments were closed by inspectors or received violations serious enough to pose an immediate public health risk. Violations in this category include evidence of pests, improper food temperature control, contaminated food-contact surfaces, or conditions that create imminent danger to public health. A follow-up inspection is required before any closed establishment can reopen, and some may face additional scrutiny or civil penalties.
Circle K Store #2706305, 1401 Coors Blvd NW. Inspected October 19 and October 21. Inspectors found a mouse infestation on the sales floor, with mice that had eaten through candy items and left droppings under shelving and in candy boxes. Droppings were also in the backroom around the three compartment sinks, and a 12 by 12 inch hole in a wall had chewed insulation. Both inspections ended with a closure and required re-inspection. A third inspection on October 22 for a separate complaint was approved.
Marielenas Bakery LLC, 640 Coors Blvd NW, Suite 3. Inspected October 21. Cockroaches, rodents, and flies were present throughout the facility, living and dead, with mouse droppings everywhere. The three compartment sink faucet had been removed, no sanitizer was made at the time of inspection, and the permit was expired. Closed and re-inspection required. The establishment is listed as closed.
Sushi Xuan, 417 Tramway Blvd NE, Suite 3. Inspected October 21. A complaint inspection found degreaser stored next to tempura battered meat, raw squid and meat scraps stored above ready-to-eat food, no hot water at the kitchen hand sink, and eleven cold-holding items above 41 degrees, including cooked shrimp at 53 degrees. Salmon served raw had no parasite destruction documentation. Closed and re-inspection required. A corrective action inspection the same day left the facility in unsatisfactory status pending re-inspection within five business days.
Madcrema, 1108 Broadway Blvd NE. Inspected October 20. A complaint inspection found an unapproved pesticide in the dish pit, dead cockroaches in the kitchen, dish pit, and restroom, and single-use containers being reused for storage. Result was unsatisfactory with re-inspection required. A corrective action inspection the same day upgraded the facility to approved after staff fixed handwashing, cleaned food contact surfaces, and remade sanitizer.
Whole Hog Cafe, 9880 Montgomery Blvd NE. Inspected October 20. A prep cook cut raw chicken and then touched beef with the same gloves. Both hand sinks in prep and on the line had no hot water, and chlorine sanitizer at the front counter measured 200 ppm. Result was unsatisfactory with re-inspection required. A second inspection the same day was approved.
Conditional Approvals
These establishments remain open but were given conditional approval following their inspection. They were found to have violations that, while not immediately dangerous, require prompt correction. Each must address the identified issues within a defined window set by inspectors. Failure to correct the violations within that timeframe triggers a mandatory re-inspection and may result in conditional status being revoked or the establishment being closed.
Golden Corral, 2701 Coors Blvd NW. Conditional on October 20 for dried soil on the meat slicer blade and heavy buildup inside the ice machine, plus raw chicken and other items without date marks. A corrective action inspection October 22 cleaned the slicer and ice machine and brought the score to approved.
Ichiban Japanese Rest., 10701 Corrales Rd, Suite 18. Conditional on October 23. A container of Evap Foam Rinse was stored next to a bag of rice, chlorine sanitizer measured above 200 ppm, and ice was dumped in the hand wash station.
La Petite Academy Inc., 5701 Redlands Rd NW. Conditional on October 22. No certified food protection manager on staff, raw beef stored next to shredded lettuce and butter, and the kitchen hand sink used for dumping juice and rinsing produce.
The Watermark at Cherry Hills, 6901 San Vicente Ave NE (Permit PT0155676). Conditional on October 21 for organic buildup in the ice machine hopper and cocktail sauce opened seven months earlier with no date mark. A corrective action inspection the same day upgraded the facility to approved after old food was discarded.
Unser Bakery LLC, 10850 Unser Blvd NW. Conditional on October 21. No certified food protection manager, bakery items in the display case holding above 41 degrees, and no hand cleanser or towels at the front hand sink.
