Closures & Major Violations
These establishments were closed by inspectors or received violations serious enough to pose an immediate public health risk. Violations in this category include evidence of pests, improper food temperature control, contaminated food-contact surfaces, or conditions that create imminent danger to public health. A follow-up inspection is required before any closed establishment can reopen, and some may face additional scrutiny or civil penalties.
999 Seafood Supermarket Inc, 5315 Gibson Blvd SE
June 26, 2026. This grocery held three permits, all of which came back Unsatisfactory. Inspectors found rodent droppings in the grocery aisles, including on bulk rice bags, and on boxed chopsticks and trash bags in back storage. Earlier violations addressed before reopening included spoiled seafood, an active pest infestation treated by a technician, a contaminated meat scale that was discarded, and unlabeled chemicals. All three permits reopened under Unsatisfactory grades with follow-up inspections required within five business days.
Amadeos Pizza & Subs, 809 98th St SW
June 22, 2026. The facility had an active cockroach infestation, with single-use cups, lids, and containers stored directly on the floor where roaches were using them for harborage. Raw shelled eggs sat above ready-to-eat romaine in the walk-in cooler, and the grease trap was leaking waste water sludge onto the floor. The person in charge could not name the six reportable illnesses. Closed, re-inspection required.
Einstein Bros Bagels, 8060 Academy Rd NE, A
June 26, 2026. Line cooks cracked raw eggs and continued preparing sandwiches without washing hands or changing gloves. The three-compartment sink delivered only 140 degree water, so quat sanitizer was being mixed with hot water alone, and the men's restroom toilet would not flush. The routine inspection returned Unsatisfactory and re-inspection is required. The facility was also issued a Conditional Approved on the same day after the ice machine was cleaned and sanitized on site.
Hilton Garden Inn, Journal Center, 5320 San Antonio Dr NE
June 24, 2026. The routine inspection returned Unsatisfactory. Sliced tomatoes, chopped green chile, and Greek yogurt held at 49 to 55 degrees in a cooler that was not maintaining temperature, and expired food dated June 15 and 16 was still in the walk-in. The ice machine had extreme black organic buildup. The person in charge corrected several priority violations on site during a corrective action visit, bringing the score back to Approved, but the permit's official routine result stands as Unsatisfactory with re-inspection required.
Hot Dog Man, 314 Iron St SW, C
June 22, 2026. Sanitizer in the spray bottle and wiping cloth container tested at zero parts per million, and the hand wash station had no soap, no towels, no sign, and was being used to store tools and dump drinks. An employee washed hands using a soiled wiping cloth from a bucket of dirty water. Closed, re-inspection required. At a June 24 corrective action visit, the operator drove away and did not allow the inspector to complete the inspection; the unit remains Unsatisfactory.
Kobe Pho, 10200 Corrales D4 Rd NW
June 22, 2026. The routine inspection returned Unsatisfactory. Front and rear handwash sinks were blocked with kitchen equipment and soiled utensils, broths and prepared meats carried no date marks, and chopped fruit, broth, and sliced onions held at 47 to 56 degrees. A same-day corrective action visit upgraded the facility to Approved after cold-holding food was discarded, the slicer was cleaned, and sinks were cleared, but the routine result stands as Unsatisfactory.
Springhill Suites, 5910 Holly Ave NE
June 23, 2026. The routine inspection resulted in a Closure. Sanitizer had not been prepared for the day and tested at zero parts per million, sliced ham and cheese carried no date marks, and the person in charge could not identify the sanitizer in use or check thermometer calibration. The facility avoided closure after correcting one priority violation on site, bringing the score to Unsatisfactory. All food employees are required to take a food safety course, and re-inspection is required.
The Paleta Bar, 12501 Candelaria Rd NE, D
June 23, 2026. The routine inspection resulted in a Closure. Butter held at 73 degrees and cotija cheese at 64 degrees, both well above safe cold-holding limits, and the can opener blade had dried food debris. Handwashing sinks behind the front counter were blocked, one holding a towel in the basin and the other by brooms and dust pans. A concurrent inspection under a second ID returned Conditional Approved. Re-inspection required.
Conditional Approvals
These establishments remain open but were given conditional approval following their inspection. They were found to have violations that, while not immediately dangerous, require prompt correction. Each must address the identified issues within a defined window set by inspectors. Failure to correct the violations within that timeframe triggers a mandatory re-inspection and may result in conditional status being revoked or the establishment being closed.
Antonio Martinez Reyes, 1725 Broadway Blvd SE
This mobile unit held hot dogs, cut tomatoes, raw hamburgers, and guacamole at 45 to 50 degrees, and the hand wash station had no towels. It needs to bring cold holding into range and provide hand drying and training records.
Guru Investments Inc, Taj Mahal Restaurant, 1430 Carlisle Blvd NE
Inspectors found ten or more dead cockroaches in basement storage and one in the kitchen, an ice machine coated with black organic matter, and undated cut vegetables and cooked chicken. It needs pest removal, an ice machine cleaning, date marking, and a certified food protection manager.
Rose Garden, 10401 Golf Course Rd NW, 103
Battered chicken, shrimp, and raw marinated beef held at 45 to 48 degrees, raw shelled eggs sat above ready-to-eat noodles, and the ice machine ceiling had black soil residue. A same-day corrective action visit cleaned the ice machine, adjusted cold holding, and separated raw from ready-to-eat food.
