Closures & Major Violations
These establishments were closed by inspectors or received violations serious enough to pose an immediate public health risk. Violations in this category include evidence of pests, improper food temperature control, contaminated food-contact surfaces, or conditions that create imminent danger to public health. A follow-up inspection is required before any closed establishment can reopen, and some may face additional scrutiny or civil penalties.
Little Lobitos, 5015 4th St NW
Inspected October 7. Inspectors found mouse droppings in the backstock room and kitchen, along with ten dead cockroaches in a trap by the reach-in coolers. The facility was closed on the spot. A complaint inspection two days later, on October 9, was marked approved, but the facility still lacked temperature logs, calibration logs, and a certified food protection manager.
NMR DBA Dunkin' Donuts, 5502 4th St NW
Inspected October 6. The main refrigeration ran at 50 degrees, holding cream cheese at 49 degrees and milk at 55 degrees, and walk-in freezer units dripped onto open ice cream and food boxes. Sanitizer buckets tested at 0 ppm, and two live roaches were found in the storage area. The facility was closed. A follow-up on October 8 upgraded it to Unsatisfactory, with food handler cards, wall and pest repairs, and freezer work still required.
Ojos Locos Sports Cantina, 2105 Louisiana Blvd NE (Permit PT0116581)
Inspected October 9. Sanitizer buckets failed to meet temperature requirements, and the main kitchen line had no sanitizer set up at all. The dish machine and racks carried heavy debris, raw meat was thawing at room temperature in a sink, and several cold-held items ran as high as 75 degrees. The facility required a closure re-inspection. A separate permit at the same address (PT0116582) was approved.
Conditional Approvals
These establishments remain open but were given conditional approval following their inspection. They were found to have violations that, while not immediately dangerous, require prompt correction. Each must address the identified issues within a defined window set by inspectors. Failure to correct the violations within that timeframe triggers a mandatory re-inspection and may result in conditional status being revoked or the establishment being closed.
Embudo Towers, 8010 Constitution Ave NE
Inspected October 6. The facility had no certified food protection manager, no food handler cards, grapes held at 55 degrees at room temperature, no hand drying at the kitchen sink, and an expired permit. A corrective action visit the next day cleared the hand drying and retraining items.
Family Dollar, 2801 Eubank Blvd NE
Inspected October 9. Inspectors found many dead cockroaches in the back storage room, with bottled drinks stored on the floor among them. Raw eggs were stored above bottled beverages, and the permit was expired with no sticker or permit posted.
Ojos Locos Sports Cantina, 2105 Louisiana Blvd NE (Permit PT0116581, first routine)
Inspected October 9. No certified food protection manager, no food handler cards, and employees switched job duties without washing hands while the prep sink was blocked. Ventilation ducts had debris buildup, and temperature logs were missing.
