Closures & Major Violations
These establishments were closed by inspectors or received violations serious enough to pose an immediate public health risk. Violations in this category include evidence of pests, improper food temperature control, contaminated food-contact surfaces, or conditions that create imminent danger to public health. A follow-up inspection is required before any closed establishment can reopen, and some may face additional scrutiny or civil penalties.
999 Seafood Supermarket Inc — 5315 Gibson Blvd SE
Inspected June 4, 2026. Three separate permits at this location were all closed. Inspectors found mice droppings throughout the warehouse, on food boxes, on storage shelving, and in storage vaults. Dead roaches were found under freezer equipment on the sales floor. Raw chicken was stored above pork and beef with bloody, dripping packaging. Spoiled seafood in a state of decay shared ice and display cases with products still for sale. Frozen fish was thawing in standing water. Multiple cold holding units failed temperature requirements: tofu at 47 degrees, eggs at 70 degrees, bean sprouts at 50 degrees. The person in charge could not demonstrate basic food safety knowledge. Wastewater pooled on the raw meat and seafood floor. Holes in walls and ceilings ranged from half an inch to six inches. The facility had no sanitizer prepared, no temperature logs, no food handler cards on file, and light bulbs throughout the walk-in coolers lacked protective covers. All three permits require re-inspection before reopening.
Limra DBA Royal Hyderabad — 5760 McMahon NW Blvd, Ste B-E
Inspected June 5, 2026. Frozen chicken was left thawing at room temperature on a milk crate. Multiple items in prep coolers exceeded safe temperatures: green paste at 48 degrees, cooked potatoes at 45 degrees, chili sauce at 51 degrees, curry chicken at 47 degrees. In the walk-in freezer, substantial condensation and ice dripped onto shrimp and other food stored directly on the floor. Cooked items in the walk-in cooler had no date markings and sat uncovered. The person in charge failed to answer questions about the six reportable illnesses, hot holding temperatures, reheating requirements, and cold holding standards. No hand drying provisions were available at the kitchen hand wash station or employee restroom. No food handler cards were on file. Closure with re-inspection required.
Conditional Approvals
These establishments remain open but were given conditional approval following their inspection. They were found to have violations that, while not immediately dangerous, require prompt correction. Each must address the identified issues within a defined window set by inspectors. Failure to correct the violations within that timeframe triggers a mandatory re-inspection and may result in conditional status being revoked or the establishment being closed.
Birria Estilo Zacatecas (DBA Los Tacos del Rey) — 600 Louisiana Blvd SE, A
Sanitizer in wiping cloth buckets and the three-compartment sink measured near zero. Ready-to-eat foods including salsas, sliced cheeses, cooked meats, and prepared trays throughout multiple coolers had no date markings. Raw shelled eggs were stored directly over bulk unprotected cilantro. The facility corrected sanitizer, labeling, and egg storage issues during the inspection and was upgraded from Unsatisfactory to Conditional.
